This form tells you about Damir Plazibat and the AscendancyCo.
By typing my name below and clicking, " I agree," I agree to the following.
1. Damir Plazibat is not a State licensed physician or surgeon. He will not diagnose illnesses, infirmities, or diseases for you. Damir Plazibat will not prescribe medicines or medical treatments for you or recommend that you discontinue drugs, medicines or controlled substances prescribed by an appropriately licensed practitioner. Damir Plazibat neither encourages nor discourages you from contacting a state licensed physician or surgeon.
2. Damir Plazibat has been practicing yoga, prayer, wellness energy healing techniques and modalities which include but are not limited to: Vinyasa Yoga, Bikram Yoga, Kundalini Yoga, Sivananda Yoga, Reiki, Spiritual Response Technique or SRT, the Thetahealing® technique, working with Shamanic techniques and medicines among other not widely known spiritual practices for the sole purpose of overall healing and transformation.The theories and practices of many of these modalities is that the human body as energy fields can be affected by subtle energies through prayer or focused intention.
3. Damir Plazibat received his Bachelor's Degree in business from The California Coast University, followed by ThetaHealing ® Certificate of Science, the Munay-Ki series of nine Empowerement rites based on the initiatory practices of the Q'ero shamans of Peru. Also received his Reiki and SRT certifications along with various Monroe Institute certifications.
4. AscendancyCo with Damir Plazibat is an alternative umbrella name for the services offered by Damir Plazibat; all restrictions and limitations apply.
5. A person may choose to meet with Damir Plazibat in an individual or group session, at which some of all of these modalities are administered and utilized of their own free will.
6. The prayer and energy healings you will receive are not licensed by Florida State and are alternative or complementary to healing arts services licensed by the state of Florida.
7. All sessions are confidential. AscendancyCo does not keep session files or client information other than scheduling.
8. Damir Plazibat is neither responsible for nor obligated to refund any reading, session or consulting payments; all readings, sessions, consulting, which are cancelled less than 24 hours before a scheduled reading require full payment regardless of whether they are rescheduled; all class deposits are non-refundable and are only transferrable to another class subject to Damir’s discretion.
9. All personal information revealed in a reading, session, consultation, workshop, or class is kept strictly confidential, unless an individual’s safety is at risk, in which case appropriate authorities will be notified. Determining this is based upon Damir’s discretion.
10. Damir Plazibat reserves the right to dismiss you from a workshop or course without reimbursement should you fail to honor the protocol of the AscendacyCo (applicable to in person sessions and classes only) .

I understand the contents of this form and voluntarily choose of my own free will to participate in meetings, classes, sessions, consultations or services of Damir Plazibat or to receive prayer or energy healings as described above.
